Objectives:

Diabetes has become a major public health problem in China. Chronic complications are the major causes of disabilities and death for diabetic patients. It is well known that good control of blood glucose, blood pressure, lipids and aspirin usage reduces the risk of complications. This proposal describes a community-hospital integrated management system for diabetic patients in Beijing, China, with the purpose of translating standard care to the real world clinical practice to optimize control of blood glucose, blood pressure and lipids. The methods in this proposal are based on the previous Beijing Community Diabetes Study (BCDS).
